Can a cited answer still be wrong?

Yes. A citation can point to a document that does not support the claim, or to an outdated passage. Evaluation should check relevance and support, not simply whether a link is present.

Should we upload every document first?

Usually not. A smaller, authoritative collection makes it easier to test access, freshness and usefulness. Expand coverage once the initial collection performs acceptably.
